Talk with a case manager
Call to talk through what your loved one needs and how case management can coordinate it, with one main point of contact for the client and his supports.
What case managers do
- Assessment. Evaluate mental health and substance use needs and build a personalized care plan.
- Coordination. Organize access to services such as therapy, medication management and treatment programs, bringing together every agency, clinician and doctor involved.
- Advocacy. Advocate for the client’s rights and needs within the healthcare and legal systems.
- Monitoring. Track progress continuously and adjust the plan as needs change.
- Support. Provide emotional support and education to clients and their families, and serve as the main point of contact for everyone involved.
- Crisis intervention. Help manage crises and emergencies related to mental health or substance use.
Why it matters
Evidence shows that comprehensive, wrap-around case management improves outcomes and long-term sustainability. By removing barriers to care and reducing outside stress, case management lets clients focus on personal growth.
Our team
Our case managers are highly trained professionals with deep experience in mental health and substance use treatment.
Who case management is for
Case management helps when recovery involves many moving parts. It is often a good fit for:
- People with co-occurring mental health and substance use conditions who see several providers
- Clients stepping down from residential treatment who need a plan for what comes next
- Anyone dealing with legal, medical, housing or employment issues alongside recovery
- Families who are overwhelmed trying to coordinate care for a loved one
- Clients who have tried treatment before and need a stronger plan this time
What working with a case manager looks like
Work begins with an assessment of where the client is today: health, treatment history, legal or financial pressures, family relationships and goals. From there, the case manager builds a care plan and becomes the single point of contact for everyone involved.
Day to day, that can mean finding the right therapist or psychiatrist, scheduling appointments, keeping providers informed, helping with paperwork, communicating with courts or employers when appropriate, and keeping the family updated so no one is left guessing.

Case management questions
What does a recovery case manager do?
A recovery case manager assesses a client’s needs, builds a personalized care plan, coordinates every provider involved, advocates for the client and keeps the family informed, so the client can focus on recovery.
How is case management different from therapy?
A therapist treats the emotional and psychological side of recovery. A case manager coordinates the whole plan around it, including treatment, appointments, legal or medical needs and communication between providers.
Can families work with a case manager?
Yes. Families are often the ones who call us first. With the client’s permission, our case managers keep families updated and help them understand how to support recovery.
Do you help with legal or court requirements?
Our case managers can help clients understand and organize court or legal requirements and communicate with the right people when appropriate. We are not a law firm and do not give legal advice.
How long does case management last?
It depends on the client. Some need intensive support for a few months after treatment, and others benefit from longer-term coordination as their needs change.
